About

TRANSITIONAL SUPPORT SERVICES (TSS) provides intensive case management and wraparound community support services to adults (18+) with co-occurring mental health and intellectual or developmental disabilities.All services support successful integration and stability within communities. Individuals gain a sense of purpose as well as the independence and knowledge to lead a meaningful life.

How to get involved

All referrals are made through North Bay Regional Center/NBRC and are sent to TSS Program Director.
