Sutter Health NAVIGATION TO WELLNESS is a collaborative program with Sutter Hospital that provides individuals with care coordination support post-hospital discharge. Our team works with individuals to provide support in gaining access and linkage to programs and services within the community. This support continues for up to 60 days after hospital discharge to ensure successful care coordination.
